Subject: Membership-based Model Announcement
Dear Loyal Patients,
As we approach the new year and the second anniversary of Monarch Health, I reflect on the significant growth we have experienced, and the meaningful relationships we have built as you have entrusted me with essential aspects of your healthcare. I am grateful for your trust and partnership.
With growth comes the need for thoughtful reflection and necessary adjustments. To continue providing the highest level of specialized, nuanced, and personalized care that we are known for, Monarch Health will transition from fee-for-service to a membership-based model effective January 1, 2025.
This shift will allow us to be fully present, ensuring each patient receives dedicated and personalized care. We understand that change can bring questions and are here to guide you through this transition. We realize that membership-based care does not work for everyone, but we view health and wellness as a journey. In keeping with this vision, we know episodic, fee-for service care model is not the optimum delivery method. The membership-based model of care will allow us to ensure that each patient can delve into their health history, fully explore current health needs, and have partnership on the path to achieving long-term health goals.
Initial consultations will still be available and the cost of the visit will serve as the membership enrollment fee should you choose to join our practice. All established patients are exempt from an initial registration fee. We will work with you and your personal needs to help decide which membership tier is best for you. For established patients, any appointments that are currently booked will still be honored through January 30, 2025 at the fee-for-service model pricing. After this date, if you do not plan to join the membership, then we are happy to discuss the secure transfer of your medical records and prescriptions.
Membership details will be available on our website in early December. Key benefits to look forward to include direct provider access via patient portal, email and text, prescription management, care coordination outside of visits, and health inquiries over text or email.
We are also excited to announce that membership-based care will allow you to work with new members of our dedicated and specialized team. In 2024, we added practice administrator Lauren Drummond, who has been an invaluable asset to Monarch Health. In 2025, we will be adding on a nurse and patient care coordinator, as well as a nutritional therapist.
Enrollment will be available through the Hint platform starting in mid-December. You will receive separate communication when the platform is live to select your membership plan, manage payments, and sign financial agreements. Current patients, including those scheduled for upcoming visits, will be given priority enrollment, as the number of memberships will be limited.
Over the past two years, I have learned invaluable lessons about the business of medicine, recovered from burnout associated with insurance-driven healthcare, and refined my practice to deliver evidence-based, compassionate, and personalized medical care. Thank you for allowing me to be part of your healthcare journey.
I look forward to continuing to partner with you in your healthcare.
