  • To continue providing the highest level of specialized, nuanced, and personalized care that we are known for, Monarch Health, LLC will transition to a membership-based model effective:

    Our transition to a membership model allows us to provide a higher level of personalized, proactive care. By moving away from the traditional fee-for-service structure, we can focus on delivering comprehensive, unhurried, and holistic care without the constraints of per-visit billing. This change will also give us the flexibility to offer more in-depth support between visits, enhancing your overall experience and access to care. We understand that change can bring questions. We are here to guide you through this transition.

    Memberships will be limited to ensure that each patient can delve into their health history, fully explore current health needs, and have partnership on the path to achieving long-term health goals.

  • Membership fees varies depending on tier of choice and level of need.

    Memberships start at $129 per month.

  • Membership includes access to a variety of services such as comprehensive wellness visits, care coordination, direct communication with our team, and personalized treatment plans.

    The membership model also offers the convenience of shorter wait times, priority scheduling, and ongoing support for managing your health, including menopause management and lifestyle counseling.

    Key benefits to look forward to include direct provider access via patient portal, email and text, prescription management, care coordination outside of visits, and health inquiries over text or email.

  • The membership fee covers services provided directly by our practice and is typically not reimbursable by insurance. However, if you receive services outside the membership structure (like lab tests, imaging, or referrals to specialists), you may still use your insurance for those.

    We can provide necessary documentation to assist with any out-of-network claims you wish to submit.

  • Honestly, it really shouldn’t.

    But, our goal is to elevate the quality and accessibility of the care you already receive. With the membership model, we can dedicate more time and resources to addressing your needs. This model fosters a more holistic approach, allowing us to proactively manage your health with greater availability and more comprehensive, personalized care.

  • We understand that the membership model may not suit everyone.

    We are accepting a limited number of fee-for-service patients.

    Patients who choose not to participate in the membership program may need to find alternative care providers. We will assist with referrals or provide guidance on how to transition your care seamlessly if needed.

  • Enrollment will be available through the HINT platform starting in mid-December. You will receive separate communication when the platform is live to select your membership plan, manage payments, and sign financial agreements.

    If you are considering enrollment as a new patient, please email membership@mymonarchhealthco.com to receive these emails and to be placed on our wait-list. Current patients, including those scheduled for upcoming visits, will be given priority enrollment.
